SERVICES |
After a person has been determined eligible for services, a Service Coordinator will be assigned to the case and develop a Service Plan based on individual needs and ways to meet these needs. A person or family member may chose to have an independent Facilitator to assist in the development of the Plan, but this is not required. If you have ongoing (Level 1) Service Coordination, the service plan will be monitored at least quarterly by the Service Coordinator for consumer satisfaction, usefulness and effectiveness of services.
Service Coordination is optional and if you feel you do not need a Service Coordinator, you can choose to receive Level 2 Service Coordination, where you will only be contacted annually.
Services are NEEDS BASED and dependent upon OPENINGS in a program area.
